Are you on stock rom? I had to go back to stock rom for the menu to come up after dialing *#22745927#

Give it a shot, I was without 4g for months as well. I actually thought something was wrong with my phone, turns out I must have messed something up when I was flashing a rom.

Make sure to backup anything important before attempting this.
After reverting back to stock dial *#22745927# and enable hidden menu, go back then dial ##786# a menu will come up with two options, "view and reset" press reset. It will ask you for your MSL# (mine was 000000 yours may be as well) when it asks you to reset phone to manufacture's default put Yes. Your phone will boot into recovery and restore stock settings. Upon rebooting your phone SHOULD connect to 4g, at least that's what mine did.

4g should work on custom rom's as well, I'm using CM 10.1.3 with no problems.
